{"id":1571,"date":"2025-04-05T12:25:07","date_gmt":"2025-04-05T12:25:07","guid":{"rendered":"https:\/\/guidepost.aztute.com\/?p=1571"},"modified":"2025-04-05T12:25:07","modified_gmt":"2025-04-05T12:25:07","slug":"%f0%9f%91%a9%e2%80%8d%f0%9f%92%bc-supervising-case-manager-guide","status":"publish","type":"post","link":"https:\/\/aztuteguidepost.azurewebsites.net\/index.php\/%f0%9f%91%a9%e2%80%8d%f0%9f%92%bc-supervising-case-manager-guide\/","title":{"rendered":"\ud83d\udc69\u200d\ud83d\udcbc Supervising Case Manager Guide"},"content":{"rendered":"<p><strong>\ud83d\udc69\u200d\ud83d\udcbc Supervising Case Manager Guide \u2013 Libertana ECM POC<\/strong><\/p>\n<p><strong>\ud83d\udd39 Role: Supervising Case Manager (SCM)<\/strong><\/p>\n<p><strong>\ud83c\udfaf Objective:<\/strong><\/p>\n<p>To assign members to Case Managers, oversee outreach, HRA, and care plan completion, and use 360 View to monitor progress across the ECM journey during the POC.<\/p>\n<p><strong>\u2705 Pre-Setup Checklist<\/strong><\/p>\n<p>Before you begin:<\/p>\n<ul>\n<li>\u2705 Ensure members have been uploaded by the <strong>Admin<\/strong> (visible in \u201cAll Member Status\u201d)<\/li>\n<li>\u2705 Case Managers are created and have correct roles assigned<\/li>\n<li>\u2705 You can see the <strong>\u201cAssign to Case Manager\u201d<\/strong> option in your dashboard<\/li>\n<\/ul>\n<p><strong>\ud83e\udded Main Screens You Will Use<\/strong><\/p>\n<ul>\n<li><strong>All Member Status<\/strong><\/li>\n<li><strong>All HRAs<\/strong><\/li>\n<li><strong>All Care Plans<\/strong><\/li>\n<li><strong>Member 360 View<\/strong><\/li>\n<li><strong>My Tasks<\/strong><\/li>\n<\/ul>\n<p>You\u2019ll find these on the left navigation panel after login.<\/p>\n<p><strong>\ud83d\udd01 Step-by-Step Workflow<\/strong><\/p>\n<p><strong>\ud83d\udd39 Step 1: Assign a Member to a Case Manager<\/strong><\/p>\n<ol>\n<li>Go to <strong>All Member Status<\/strong><\/li>\n<li>Search for the member using their <strong>name, CIN<\/strong>, or filter by <strong>Program = ECM<\/strong><\/li>\n<li>On the right side of the row, click <strong>Action \u2192 Assign to Case Manager<\/strong><\/li>\n<li>A popup appears \u2013 choose the Case Manager from the dropdown<\/li>\n<li>Click <strong>Assign<\/strong><\/li>\n<\/ol>\n<p>\u2705 Result:<\/p>\n<ul>\n<li>Member is now assigned<\/li>\n<li>Case Manager receives task + notification<\/li>\n<li>Assignment is reflected in the dashboard<\/li>\n<\/ul>\n<p><strong>\ud83d\udd39 Step 2: Monitor Outreach Completion<\/strong><\/p>\n<ol>\n<li>Return to <strong>All Member Status<\/strong><\/li>\n<li>Use filters:\n<ul>\n<li><strong>Program = ECM<\/strong><\/li>\n<li><strong>Outreach Status = Pending \/ Completed<\/strong><\/li>\n<\/ul>\n<\/li>\n<li>Once outreach is done, status updates to <strong>Completed<\/strong><\/li>\n<li>Click <strong>Action \u2192 View Outreach<\/strong> to inspect the submitted details<\/li>\n<li>Confirm whether outreach was marked <strong>Successful<\/strong><\/li>\n<\/ol>\n<p>\u2705 Successful outreach auto-triggers an HRA task for the Case Manager<\/p>\n<p><strong>\ud83d\udd39 Step 3: Track HRA Progress<\/strong><\/p>\n<ol>\n<li>Go to <strong>All HRAs<\/strong><\/li>\n<li>Filter by Program = ECM or Case Manager name<\/li>\n<li>Find the HRA entry for your assigned member<\/li>\n<li>Check the <strong>Status<\/strong> column:\n<ul>\n<li>\u201cIn Progress\u201d means it has been started<\/li>\n<li>\u201cCompleted\u201d means it\u2019s done<\/li>\n<\/ul>\n<\/li>\n<li>Click <strong>View<\/strong> or <strong>Edit<\/strong> to review<\/li>\n<\/ol>\n<p>\ud83e\udde0 Tip: If it still says \u201cCreated,\u201d follow up with the Case Manager<\/p>\n<p><strong>\ud83d\udd39 Step 4: Monitor Care Plan Completion<\/strong><\/p>\n<ol>\n<li>Open <strong>All Care Plans<\/strong><\/li>\n<li>Use filters to narrow by <strong>Status<\/strong> or <strong>Date<\/strong><\/li>\n<li>Click on a Care Plan entry to view:\n<ul>\n<li>Goals<\/li>\n<li>Interventions<\/li>\n<li>Dates<\/li>\n<li>Status<\/li>\n<\/ul>\n<\/li>\n<li>You can also <strong>Download as PDF<\/strong> for offline tracking<\/li>\n<\/ol>\n<p>\u2705 By the end of the POC, the Care Plan should be either <strong>In Progress<\/strong> or <strong>Completed<\/strong><\/p>\n<p><strong>\ud83d\udd39 Step 5: Use Member 360 for Oversight<\/strong><\/p>\n<ol>\n<li>Go to <strong>All Member Status<\/strong><\/li>\n<li>Locate the member and click <strong>Action \u2192 360 View<\/strong><\/li>\n<li>Review the full member journey:\n<ul>\n<li>Demographics &amp;
